Final Evaluation of Student (MPH)

Thank you for your time and guidance in working with your MPH intern. We appreciate your willingness to assist in our internship program by providing your intern with a well-rounded, practical field experience. Please use the following checklist and questions to evaluate the student based on the Essential Services of Public Health, and the student's level of attainment on the Foundational Competencies developed by the Council on Education for Public (CEPH) as well as the MPH Concentration Competencies developed by the faculty in the IU School of Public Health-Bloomington (SPH-B). The Essential Public Health Services Provide the fundamental framework for the National Public Health Performance Standards Program instruments, by describing the public health actives that should be undertaken in all communities. The CEPH Foundational Competencies are informed by the traditional public health core knowledge areas, (biostatistics, epidemiology, social and behavioral sciences, health services administration and environmental health sciences), as well as cross-cutting and emerging public health areas.

Please discuss this evaluation with the student during the exit interview.
